The research achievements of our founder, Dr. Joel Swai, serve as exemplars of resilience, integrity, and academic excellence in the face of systemic barriers.

Dr. Joel Swai is a licensed Tanzanian physician and researcher currently serving at Epistatearch. With over six years of clinical and research experience across Tanzania, China, and Canada, he has contributed to internationally peer-reviewed publications in internal medicine, epidemiology, biostatistics, clinical research, and meta-analysis—many of which are indexed in leading academic databases.
Dr. Swai is committed to advancing medical research in developing regions, leveraging his expertise to promote evidence-informed health policies grounded in locally generated and analysed data.

Peer-Reviewed Publications
Dr. Joel Swai has authored several peer-reviewed articles across internal medicine, epidemiology, biostatistics, clinical research, and meta-analysis. These works are indexed in leading international academic databases and reflect rigorous methodologies and scholarly impact.

Experience Navigating Ethical Clearance and Publication in Tanzania
Dr. Swai has firsthand experience with the fragmented and resource-intensive process of obtaining ethical approval in Tanzania. For one of his published studies, he successfully navigated approvals from multiple bodies—including an institutional review board, a zonal ethics committee, and the National Institute for Medical Research (NIMR)—each with its own protocols, fees, and review timelines.
After securing ethical clearance (Ref: NIMR/HQ/R.8a/Vol.IX/3545), he conducted and completed the study. However, post-study requirements to resubmit findings to all approval bodies before publication proved both time-consuming and discouraging. The complexity of this process led to the abandonment of a subsequent project, despite having received prior ethical approval (NIMR/HQ/R.8a/Vol. IX/3867).
Dr. Swai’s experience underscores the importance of maintaining ethical rigor while advocating for a streamlined, centralized review process that supports research productivity without compromising standards.

International Research Experience
In contrast, Dr. Swai’s collaborations in China—working with researchers from China and Syria—were conducted under a more cohesive and efficient ethical review framework. These efforts resulted in the successful publication of three original research articles, demonstrating how a unified system can accelerate the research process while maintaining compliance and quality.

Systematic Reviews and Meta-Analyses
Dr. Swai has also contributed to the global evidence base through the publication of four systematic reviews and meta-analyses. These studies involved exhaustive literature reviews and data synthesis, showcasing methodological rigor and long-term commitment. While current platforms like PROSPERO support transparency and consistency, Epistatearch recognizes the transformative potential of AI to streamline and enhance the review process in the future.

Letters and Commentaries
A Letter to the Editor, co-authored with Taiwanese colleagues, addressed an error in a published figure. This publication—his most cited to date—demonstrates both academic diligence and the value of international collaboration.
In a published Commentary, Dr. Swai expanded on a prior study, responding to the authors’ call for further analysis. While this approach proved effective, it highlighted the benefits of integrated collaboration earlier in the research lifecycle to reduce redundancy and improve scientific impact.

Pre-AI Achievements
All of Dr. Swai’s publications were completed prior to the mainstream adoption of AI in research workflows, with his most recent article published in September 2021. These achievements exemplify the commitment, discipline, and methodological expertise required in traditional research environments. Moving forward, Epistatearch aims to build on this foundation by integrating AI to enhance research efficiency, accuracy, and output across developing regions.
